Participant Waiver

RELEASE AND WAIVER OF LIABILITY AGREEMENT: By completing this form I acknowledge that I have voluntarily opted to participate in the Point Clark Lighthouse tour that includes ascending and descending the 114 steps of the Lighthouse tower.

I AM AWARE THAT THESE ACTIVITIES MAY BE HAZARDOUS, AND THAT I COULD BE SERIOUSLY INJURED. I AM VOLUNTARILY PARTICIPATING IN THESE ACTIVITIES WITH KNOWLEDGE OF THE DANGER INVOLVED, AND AGREE TO ASSUME ANY AND ALL RISKS OF BODILY INJURY OR PROPERTY DAMAGE, WHETHER THOSE RISKS ARE KNOW OR UNKNOWN.

I agree that I will not hold the Township of Huron-Kinloss, any employees, volunteers, agents or assign, nor any sponsor and partners for this event, repsonsible for any damages or bodily injury to me or others that may occur due to my participation in this activity.

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABITLIY AND A CONTRACT BETWEEN MYSELF AND THE TOWNSHIP OF HURON-KINLOSS, ANY EMPLOYEES, VOLUNTEERS, AGENTS OR ASSIGNS, SPONSORS AND PARTNERS AND SIGN IT OF MY OWN FREE WILL.

If signed by a parent or guardian: I verify that the dangers of the activites and the significance of this Release and Waiver were explained to the Participant and the participant understood them.

MUNICIPAL FREEDOM OF INFORMATION AND PRIVACY ACT

In accordance with the Municipal Freedom of Information and Privacy Act, personal information is collected under the Authority of the Municipal Act, R.S.O. 1990, c.M45, as amended, and will only be used for the purpose of maintaining adequate records of participation in the tour and for medical information when required in emergency situations.

REFUND POLICY

There are no refunds for any tour not completed, except where the tour guide deems it unsafe to continue the tour.
